The project aims to analyze the experience of ASL Caserta, which was the first in the Campania Region to undertake a digital transformation process for the management of chronic patients. The aim is to highlight the specificities of the chronic care system, based on an advanced information system capable of integrating the patient’s path and clinical data, optimizing the clinical-organizational management of chronic diseases, and increasing efficiency in the use of healthcare resources.
